My Ford Explorer looks like a semi-trailer wheel 18. What is the problem?

When the handle my 97 'Ford Explorer, which sounds like an 18-wheeler starter. Is strong and growing, is a noise that resembles a semi-trailer idling. Do you know what could be wrong with my car?
